I understand that if at any time my adopted cat escapes and is missing that I will contact KKHR so that they can provide assistance, advice and resources to help the cat return home safely.

I understand that KKHR cannot guarantee the health of its cats & kittens. KKHR provides veterinary care throughout their time at KKHR but as with many shelter cats and kittens, there can be latent health issues that arise soon after pet is adopted. Especially with kittens with undeveloped immune systems there can be issues that appear when cat/kitten is in a new environment dealing with new people/other pets/unfamiliar surroundings. Any veterinary costs associated with these issues will be adopter’s responsibility and are part of ongoing pet ownership. You must be prepared for additional costs with a new pet.

I understand KKHR has the right to reclaim any cat(s) not spayed or neutered by approx. 5-6 months old. If you have adopted a kitten younger than 5 months old it has NOT yet been fixed yet and must be done so by the time they are 6 months and confirmation of that provided to KKHR.

I understand that if KKHR has the right to reclaim any animal(s) if KKHR becomes aware of any falsification of this document or the adoption application document following the adoption.

I understand KKHR has the right to request for a home visit pre or post adoption/receipt of any animals(s). Refusal of this may be grounds for refusal of adoption or reclamation of any animal(s). We will consider this on a case by case basis.

I agree to NOT declaw the cat and if clawing becomes an issue to contact KKHR for assistance

I agree to keep cat(s) INDOORS AT ALL TIMES with the following exceptions: on a screened-in porch, with supervision on a cat harness, in a yard with a cat-proof fence (http://purrfectfence.com, http://catfencein.com)

I understand KKHR has the right to reclaim any cat(s) if we, after adoption, find that the cat has been allowed to become an outdoor cat (this excludes screened in patios and cat-proof fencing).

I understand KKHR has the right to reclaim any cat(s) if we, after adoption, find the cat is living in unsafe conditions. This can include, but is not limited to, unsanitary living environment, hoarding (of possessions and/or animals).

I agree to provide quality food, fresh water, shelter, humane treatment, and veterinary care to my new pet(s) and I will comply with all state and local ordinances concerning pet licensing and vaccinations

I understand that cats require a minimum of an annual exam, vaccinations, blood work, plus additional care should cat(s) become ill or injured. Estimated annual expenses for young, healthy cat being $150-250 & a geriatric cat $300 or more. I agree to provide all veterinary care including vaccinations for the life of the cat and understand that KKHR has the right to reclaim any cat(s) not seeing a vet regularly.

I will NEVER relinquish cat(s) to a shelter of any kind. I will not attempt to otherwise rehome the animal privately. If I am unable to care for the cat(s) for any reason, I promise to return animal(s) to KKHR to find an appropriate home with KKHR approval. If I have recommendations on a new home (family or friends), that will be considered, and they would need to go thru the KKHR application process.

I hereby grant permission for any veterinary office(s) I have been involved with currently or in the past to discuss information related to my pet ownership and care with any KKHR representative

I accept responsibility and take possession of the cat(s) identified in this agreement and hereby release Kitty Kat Haven & Rescue from all liability of injury or damages to persons, animals, or property caused in the future by this animal(s). I affirm that the information on this application is true and complete to the best of my knowledge. I understand that signing this agreement constitutes entering into a legal and binding contract between myself and KKHR with respect to animal(s) adopted from KKHR.
